Your referral (a referral is not essential, but is recommended)
Your Medicare Card
Your Health Insurance Card (if applicable)
Your New Patient Form - completing this form and emailing it to us prior to your appointment will save you time when arriving at our rooms. Download the form by clicking here. If it is not possible for you to complete it prior to arrival, just come a few minutes early so you can do it at reception.
You may also like to bring any other information you think is relevant, such as:
Written information regarding your relevant medical history, in particular prior biopsy reports, operation reports and a list of current medications.
Any scans or x-rays relating to your consultation. If possible, please bring your scans on USB, and bring a copy of the report.
